People with Intellectual and Developmental Disability (PWIDD), are living with IDD conditions and multiple co-morbidities resulting in shorter life expectancy. 

PWIDD would benefit from and are deserving of equitable access to a high quality Palliative Approach to Care. 

Many existing palliative care frameworks, guides, and associated tools lacked IDD specificity to better meet the person-centred needs of the individual, circle of support, and range of provider care partners.

The IDD Palliative Care Resource Guide and Toolkit, developed in Ontario, is intended to be an IDD specific supplement used with current gold standard palliative care models. 

The Toolkit includes IDD specific tools to aid providers cross sector (developmental services, healthcare, and palliative care specialists) in earlier identification of needs, screening and assessment, care team integration, and proactive care planning and delivery.
